Medical Preparation

☐ Complete pre-therapy labs (CBC, liver, kidney, viral panels, HLA typing)
☐ Confirm specialist consultations (hematology, cardiology, infectious disease, psychology)
☐ Update vaccinations and obtain dental clearance
☐ Discuss mental health readiness and support options
☐ Schedule baseline imaging or organ function tests (ECHO, MRI, etc.)

Caregiver & Support Plan

☐ Identify your primary caregiver (name/contact)
☐ Arrange backup caregiver or transport support
☐ Discuss job or school leave (FMLA, accommodations)
☐ Set up daily communication plan for updates

Fertility Preservation & Family Planning

☐ Ask your hematologist for a fertility preservation referral
☐ Schedule consultation with a reproductive endocrinologist
☐ Explore sperm/egg/embryo banking options and associated costs
☐ Request insurance pre-authorization for fertility procedures
☐ Discuss family planning goals with your care partner or counselor
☐ Obtain and store all documentation of preserved materials safely

Insurance & Financial

☐ Verify coverage for treatment, hospitalization, and follow-up care
☐ Request pre-authorization from your insurance provider
☐ Apply for patient assistance or travel grants (ask patient navigator/social worker for resources)
☐ Create a folder for all cost estimates, receipts, and coverage letters

☐ Set aside emergency funds for unexpected expenses (food, transport, lodging)

Emotional Readiness

☐ Schedule a mental health or counseling check-in with your health provider
☐ Speak with a patient/donor/caregiver who has been through the process (ask questions)
☐ Connect with a peer through our  Survivor Health Match
☐ Write down your top 3 hopes and dreams after treatment

Hospital Needs & Packing List

☐ Insurance card, ID, and treatment paperwork
☐ Medication list and allergy details
☐ Phone, charger, and headphones
☐ Comfortable clothes (loose-fitting, easy to change)
☐ Journal, books, or activities for downtime (game consoles, coloring books)
☐ Toiletries and personal comfort items (lip balm, bonnet or durag cap, lotion, blanket, socks)
☐ Binder or folder for test results, care notes, and discharge instructions
